The Air Force's Exceptional Family Member Program is designed to provide support to military family members with special medical and educational needs. EFMP Services include a variety of medical, family support and personnel functions. All three EFMP functions work toward a common goal of providing comprehensive and coordinated support to families.

EFMP-Medical is a relocation process designed to proactively identify family members who have special needs in order to ensure specialized medical/educational services are available as families move in conjunction with their sponsor’s PCS. It ensures a smooth transition and can assist with care coordination when necessary.

EFMP - Family Support function is provided by the Airman & Family Readiness Centers to enhance the quality of life of family members of active-duty sponsors who have special needs. Services include, but are not limited to on- and off-base information and referral resources, parent training, support groups, relocation assistance, financial management and school information.

EFMP – Personnel processes EFMP Reassignments when services for family members are not available in the local or projected locations and Deferment packages.

Enrollment in the Exceptional Family Member Program is mandatory for those who meet criteria IAW DODI 1315.19 and AFI 40-701. Criteria includes but is not limited to:

  • Family member requiring specialized medical care more than once annually
  • Family member with Asthma requiring daily/regular use of medications OR has been hospitalized or seen in the ER within past year.
  • Family member with cancer diagnosis within last 5 years
  • Long-term (more than 6 months) chronic and/or recurring mental health diagnosis
  • Special education services/has an IEP
  • Early intervention services, i.e., speech therapy, physical therapy, occupational therapy, etc.
  • Medical equipment needs, i.e., wheelchair, walker, hearing aids etc.

Family Member Relocation Clearances/Medical Clearances for Families PCSing

  • Mandatory for all families PCSing OCONUS
  • Mandatory for all Special Needs family members PCSing CONUS
  • Sponsor cannot get travel orders until process is complete
  • Contact your servicing EFMP office for needed forms & instructions
  • Start 4-6 months before your Report No Later Than Date (RNLTD)
